TurboTides Evaluation License Request

Use this form to complete the TurboTides Software License Form for an evaluation license.

Fill the company, site, software configuration, computer information, and signature details exactly as they should appear on the generated PDF. Evaluation licenses are node-locked, so the computer TXT files are required when available.

Company Information

Enter the legal licensee details and primary contact for the evaluation request.

Use the company name that should appear on the license.
Enter the main address for the licensee organization.
Add street-level address details if separate from the address above.
Example: Bangalore, Karnataka.
Example: India / 560054.
This person will be contacted for license clarification.
Add a direct phone number where possible.
Use the email address that should receive license communication.
Section 1: Licensee Site Information

Enter the site or location where TurboTides will be installed and evaluated. Add a second site only if the evaluation covers more than one location.

Usually the office, lab, or computer location for the evaluation.
Leave blank if there is only one evaluation site.

Check every TurboTides application area that should be included in the evaluation request.

Evaluation licenses are node-locked. Enter the number of seats and requested evaluation term.

Enter how many users or computers need evaluation access.
Requested license start date.
Requested license end date.
Section 4: Computer Information

Run these commands on the computer where TurboTides will be installed, then upload the two generated TXT files.

  1. Open a Windows Command Prompt on the target computer.
  2. Run ipconfig/all > user_name1.txt
  3. Run wmic csproduct > user_name2.txt
  4. Upload both TXT files below. TXT files must be 64 KB or smaller.

Section 5: Additional Terms and Signature

Review and accept the declaration from the TurboTides license form before submitting.

Enter the authorized requester's printed name.
Enter the requester's job title.
Date this request is submitted.
Signature Choose one option: draw your signature here or upload a signature image.
Upload a clear JPG signature for direct PDF placement. PNG is accepted and converted when the server supports image conversion.
